Yeah, the paths actually look fine to me, however those folders should be there:

USER-FILES/9-chris.walsh-Nottingham/media/

and the file

USER-FILES/9-chris.walsh-Nottingham/preview.xml

is where the data is loaded from to build the tree in the white left-hand panel...

The fact that it isn't finding that is probably causing an exception that is causing the Insert menu to not load either...

You'll need to check the permissions for the USER_FILES folder. Is it definitely writable?

I recently setup xerte on Windows Server and had a terrible issue too with identifying the correct user to give permissions too - from memory it is the IUSR one but I think I was finding 2 and it was the other one... or something like that...

I'd double check there first. I'll access our install to see what the issue was...

Can you try to give Everyone FULL access to USER-FILES? Just to see whther that works?

John, this is an Apache server running on Windows...

ACL permissions on USER-FILES was my first check.
Checking them again....

The folder:
C:\Apache24\htdocs\zone2.derbylearn.net\moodle\xertetoolkits\USER-FILES
contains standard inherited permissions and also:
Type | Name | Permission | Inherited From | Apply To
Allow | Everyone | Modify | <not inherited> | This folder, subfolders and files

Checking what Modify actually consists of:
Modify = Everything except { Full Control, Delete Subfolders and Files, Change Permissions, Take Ownership }. Amongst other things it does include { Delete, List Folder / Read Data, Create Fiels / Write Data, Create Folders / Append Data }

I am going to add the IUSR account and see if that makes a difference.
It is clear to me now (from the JS view source) that the creation process is the point of failure so I have to test by creating LOs and not going back into existing ones.

Hi Tom/John,

I upped Everyone permissions from Modify to Full Control. No Joy.
I checked the user account that Apache Service was running as (LOCAL SYSTEM) so I added Full Control permissions to "LOCAL SERVICE" to the USER-FILES folder. Still no joy.
We're running Windows Server 2003.

I will reboot the VM later after activity will have fallen and see if that kicks the new permissions into gear. Thanks very much for you help guys.
