The alt attribute appears to be missing for the icon on a xerte project.
See attached.
Is there anything we can do about it in our installation?

Also, when we use the accessibility settings to invert colours, the Play/Pause text and Toggle text is overlapping on the audio bar, also shown in attachment. Any advice on how to fix this would be welcome!

Re the ALT text:
on the icon that shouldn't be the case but I can replicate that here so will investigate that and we'll fix that in a future update. In the meantime you could add Alt text or title text or both by adding an optional script property to the learning object icon (1st icon in the tree) and then add something like the following:
$(".x_icon").attr("Alt","University of York logo")
$(".x_icon").attr("Title","University of York logo")

Re the audio bar text:
I can't replicate that. Can you provide a link to a specific example where this happens? Does that example have custom css added?

As far as I can see I fixed that issue with the narration bar text on 7th Oct 2018 but perhaps that didn't make it into a public release until after your version which is 3.7.14. IT's definitely fixed in 3.8x so I would suggest upgrading your installation because there are other fixes too.

However in the meantime add a styles optional property to the LO icon and add the following css:
#x_footerBlock .mejs-controls .mejs-button button {
    font-size: 0 !important;
    height: 16px !important;
    width: 16px !important;
}
