Questions are not showing up on interactive video

Hello, I have created an activity with an interactive video that contains 8 questions (I have timestamped them, have done this before without issues) but this time only 3 of the 8 questions appear. I have tried to delete and create the activity again but I keep on getting the same issue. I have attached two screenshots for illustration purposes. Can anyone help me solve it?

I do see that you are using 030 as the sync point. Please not that the sync points are in seconds.
